FNP – Family Nurse Practitioner
Drakos Family Urgent Care is looking for a skilled Family Nurse Practitioner to join our team! In this position, you will play a crucial role in providing advanced healthcare services to our patients and supporting our mission to deliver innovative healthcare solutions. Your expertise in diagnosing and treating medical conditions, coupled with your patient-centered approach, will be instrumental in enhancing patient outcomes and promoting overall wellbeing.
Essential Duties and Responsibilities
The essential functions include, but are not limited to the following:
- Conduct comprehensive health assessments, including medical history reviews, physical examinations, and diagnostic tests.
- Diagnose and treat acute and chronic illnesses and provide appropriate medical interventions.
- Develop individualized treatment plans for patients, including prescribing medications, therapies, and lifestyle interventions.
- Monitor patient progress and adjust treatment plans as necessary for optimal health outcomes.
- Educate patients and their families about preventive healthcare measures, disease management, and healthy lifestyle practices.
- Provide guidance on managing chronic conditions and supporting patients in achieving their health goals.
- Collaborate with physicians, specialists, and other healthcare professionals to coordinate patient care and ensure continuity of care.
- Consult with interdisciplinary teams to develop and implement healthcare programs.
- Maintain accurate and up-to-date patient records, including medical histories, examination findings, treatment plans, and progress notes.
- Adhere to regulatory requirements and maintain patient confidentiality.
- Participate in quality improvement initiatives to enhance patient care outcomes and promote evidence-based practices.
- Comply with healthcare regulations, standards, and policies.
- Advocate for the well-being of patients, ensuring their preferences and concerns are addressed in their healthcare journey.
- Serve as a resource for patients in navigating healthcare services and making informed decisions.
Minimum Qualifications (Knowledge, Skills, and Abilities)
- Master’s or Doctoral degree in Nursing (MSN/DNP) with specialization as a Family Nurse Practitioner.
- Current and unrestricted Nurse Practitioner license in New York State
- Ability to complete direct payer credentialing process
- Certification as a Family Nurse Practitioner.
- Proven experience as a Family Nurse Practitioner, preferably in primary care or a related healthcare setting.
- Strong clinical skills and the ability to independently assess and manage patients.
- Excellent communication and interpersonal abilities to interact with patients, families, and healthcare teams.
- Empathy and a patient-centered approach to healthcare delivery.
- Ability to work in a collaborative, interdisciplinary environment.
- Proficiency in using electronic health record (EHR) systems for documentation.
- Commitment to continuous learning and professional development.

- While performing the duties of this position, the employee is regularly required to talk or hear.

- The Family Nurse Practitioner may need to assist patients with mobility issues or reposition them during examinations or treatments. This requires the ability to lift or support patients when necessary.
- The employee frequently is required to use hands or fingers for typing, data entry, and filing.
- Clear vision is necessary for reading and analyzing data, reports, and documents.
- The noise level in the work environment is usually low to moderate.
- Adherence to safety protocols, including wearing personal protective equipment (PPE), may be required to handle potentially hazardous substances and maintain a safe working environment.
Job Types: Full-time, Part-time
Pay: $105,000.00 – $130,000.00 per year
