After your visit with our provider, you will be taken to the checkout area to receive a record of your visit and treatment discharge instructions.

After your visit, you will receive a follow-up phone call to check on your recovery, and to see if you have any questions.

Children under age 18 must be accompanied by a parent or legal guardian. Any other accompanying adult must have a notarized statement authorizing treatment, signed by the parent or guardian.
